Aging Skin

Our skin changes as we age.

These changes are slow and continuous, starting in our twenties.

What Happens?

The production of collagen and elastin, the proteins that give skin its strength and elasticity, slows down. This leads to the skin losing its shape and starting to sag.

The deep layers of fat that give skin its plump fullness starts to disappear, leading to thinner skin and hollowed areas near the eyes, and cheeks. This can also be seen on the hands where the loss of fat makes the tendons and blood vessels more noticeable.

The skin develops fine lines, wrinkles and darkened spots from years of sun exposure. And the loss of pigment leads to paler skin and gray or white hairs.

Each Person is Different

Each person’s skin appears to age at a different pace. Some people inherit skin that looks ten years younger. There are also lifestyle choices that can make our skin age even faster than normal. This includes over-exposure to sunlight (UV radiation) and smoking.

Just a few minutes of sun exposure each day can cause noticeable changes to the skin over time. The term photoaging is used to describe this sun-related skin damage. In sunny climates, photoaging may be seen in people as early as in their twenties.

To minimize the premature aging of the skin, protect your skin from sun exposure. If you are a smoker, you may see improvement in your skin after quitting, even if you have smoked for many years.

Treatments for Aging Skin

If you are bothered by visible signs of aging, a variety of treatments are available to improve your skin’s appearance.

Over-the-counter products, such as moisturizers and “cosmeceuticals,” products containing agents designed to reduce the signs of aging, such as alpha-hydroxy acids or antioxidants.

Topical retinoids have been shown to reduce fine wrinkles, splotchy darkened areas, and signs of chronic sun exposure.

Botulinum toxin (Botox) is injected into targeted facial muscles to improve the appearance of fine lines and wrinkles.

Dermal fillers (collagen, Restylane, Juvederm) are substances that are injected into the skin to make it look fuller. This is also called soft-tissue augmentation.

Dermabrasion uses a rapidly rotating brush to remove the top layer of so that a new, smoother layer grows in its place.

Chemical peels use a chemical to remove the damaged outer layers of skin, exposing new layers of skin below for an improved appearance.

Laser resurfacing helps correct wrinkles and age spots through use of a laser. The laser light causes the skin to generate new tissue that makes the skin fuller and more youthful appearing.

Microdermabrasion gently scrapes away the top most layer of damaged skin to trigger the growth of new layers.

These treatments may be used alone, or in combination with others for a more complete skin care regimen. For instance, someone may first receive Botox and a dermal filler, followed by ongoing use of a topical retinoid.

What Are Varicose Veins?

Varicose veins are swollen and twisted veins that are visible just under the surface of the skin. They appear most commonly in the legs, but also can develop in other parts of the body.

Veins are blood vessels that carry blood from the tissues of the body to the heart. In the heart, blood is pumped to the lungs to pick up oxygen. The oxygen-rich blood is then pumped out to the body through the arteries. From the arteries, blood flows through tiny blood vessels called capillaries, where it gives up its oxygen to the body’s tissues. The blood then returns back to the heart through the veins to pick up more oxygen.

Veins have one-way valves that help to keep the blood flowing toward the heart. When the valves don’t work well, blood backs up and pools in the veins. This causes them to swell and become varicose veins.

Varicose veins usually don’t cause medical problems. On occasion, they require treatment for pain, skin problems, blood clots, or other complications. People may choose to have cosmetic treatment to improve the appearance of varicose veins.

What Causes Varicose Veins?

Veins, especially those in the legs, have to pump the blood “uphill” to the heart, against gravity. Inside the veins are one-way valves that help with pumping action and prevent blood from flowing backward. These valves allow blood to flow in only one direction, toward the heart. Varicose veins develop when the valves become weakened, damaged, or don’t work well.

Weakness in the valves may be due to weakness in the walls of the veins. This weakness tends to be associated with certain factors, including increasing age, a family history of varicose veins, or high pressure inside veins due to overweight or pregnancy.

When the walls of the veins are weak, they lose their normal elasticity, like an overstretched rubber band. This makes them longer and wider and causes the flaps of the valves to separate. Blood is then able to flow backward through the valves, filling the vein and stretching it even more. The vein becomes enlarged, swollen, and often twisted trying to squeeze into its normal space.

The illustration shows the location of leg veins, which can become varicose veins. Figure A shows a normal vein with working valves and normal blood flow. Figure B shows a varicose vein with deformed valves, abnormal blood flow, and thin, dilated (stretched) walls.

What are genital warts?

Genital warts are a sexually transmitted disease (STD) caused by human papillomavirus (HPV).

Human papillomavirus is the name of a group of viruses that includes more than 100 different types or strains. Some of these cause the common type of warts that often appear on people’s hands. More than 30 of these viruses can infect the genital area of men and women, including the skin of the penis, vulva, or anus, and the linings of the vagina, cervix, or rectum.

Most people who become infected with HPV will not have any symptoms and will clear the infection on their own.

Some of these viruses are called "high-risk" and may cause abnormal Pap tests. They may also lead to cancer of the cervix, vulva, vagina, anus, or penis. Others are called "low-risk" and may cause mild Pap test abnormalities or genital warts. Genital warts are single or multiple growths or bumps that appear in the genital area.

How common is HPV?

Approximately 20 million people are currently infected with HPV. At least 50 percent of sexually active men and women acquire genital HPV infection at some point in their lives. By age 50, at least 80 percent of women will have acquired genital HPV infection. About 6.2 million Americans get a new genital HPV infection each year.

How do people get genital HPV infections?

The types of HPV that infect the genital area are spread primarily through genital contact. Most HPV infections have no signs or symptoms; therefore, most infected persons don’t know they are infected and can unknowingly transmit the virus to a sex partner. Rarely, a pregnant woman can pass HPV to her baby during vaginal delivery. A baby that is exposed to HPV very rarely develops warts in the throat or voice box.

What are the signs and symptoms of genital HPV?

Most people who have a genital HPV infection do not know they are infected. The virus lives in the skin or mucous membranes and usually causes no symptoms. Some people get visible genital warts, or have pre-cancerous changes in the cervix, vulva, anus, or penis. Very rarely, HPV infection results in anal or genital cancers.

Genital warts usually appear as soft, moist, pink, or flesh-colored swellings, usually in the genital area. They can be raised or flat, single or multiple, small or large, and sometimes cauliflower-shaped. They can appear on the vulva, in or around the vagina or anus, on the cervix, and on the penis, scrotum, groin, or thigh. After sexual contact with an infected person, warts may appear within weeks or months, or not at all.

Visible genital warts can be removed by medications the patient applies, or by treatments performed by a health care provider. Some individuals choose to forego treatment to see if the warts will disappear on their own. No treatment regimen for genital warts is better than another, and no one treatment regimen is ideal for all cases.

How is genital HPV infection diagnosed?

Genital warts are diagnosed by visual inspection. Most women are diagnosed with HPV on the basis of abnormal Pap tests. A Pap test is the primary cancer-screening tool for cervical cancer or pre-cancerous changes in the cervix, many of which are related to HPV. Also, a specific test is available to detect HPV DNA in women. The test may be used in women with mild Pap test abnormalities, or in women over 30 years of age at the time of Pap testing. The results of HPV DNA testing can help health care providers decide if further tests or treatment are necessary.

No HPV tests are available for men.

What are the treatments for HPV?

There is no cure for HPV infection, although in most women the infection goes away on its own. Treatments address changes in the skin or mucous membrane caused by HPV infection, such as warts and pre-cancerous changes in the cervix.

The majority of patients have fewer than 10 genital warts. These respond to various treatments.

Your doctor or health care provider will recommend a treatment based on the size of the warts, the number of warts, their location, and your personal preferences.

Most people require a course of therapy rather than a single treatment.

Warts on normal dry skin, such as the vulva or penis, are referred to as external warts. Warts on moist mucosal linings, such as inner part of the vagina, are called internal warts. These two types of warts are treated differently.

At-home treatment of genital warts

Aldara (imiquimod) Cream, 5%
Podofilox (0.5% solution or gel)

Professional treatment of genital warts

Cryotherapy with liquid nitrogen “freezes” off the warts. Repeat applications every 1–2 weeks.
Podophyllin is a resin that is applied to each wart and allowed to air dry. After several hours it should be washed off to prevent side effects. The treatment can be repeated weekly, if necessary. The safety of podophyllin during pregnancy has not been established. Not recommended for internal warts.
Trichloroacetic acid (TCA). A small amount should be applied only to the warts and allowed to dry, at which time a white “frosting” develops.
Surgical removal options include tangential scissor excision, tangential shave excision, curettage, and electrosurgery.
Laser surgery uses a focused beam of intensive light to treat the warts, while leaving surrounding skin intact.

Genital warts should only be treated with physician-recommended medications. Over-the-counter remedies for common warts are not safe to use in genital HPV infections.

What is the connection between HPV infection and cervical cancer?

All types of HPV can cause mild Pap test abnormalities that do not have serious consequences. Approximately 10 of the 30 identified genital HPV types can lead, in rare cases, to cervical cancer. Research has shown that for most women (90 percent), cervical HPV infection becomes undetectable within two years. Persistent infection with high-risk types of HPV is the main risk factor for cervical cancer.

A Pap test can detect pre-cancerous and cancerous cells on the cervix. Regular Pap testing, careful medical follow-up, and treatment (if necessary) help ensure that pre-cancerous changes in the cervix caused by HPV infection do not develop into life-threatening cervical cancer. The Pap test used in U.S. cervical cancer screening programs is responsible for greatly reducing deaths from cervical cancer. Most women who develop invasive cervical cancer have not had regular cervical cancer screening.

How can people reduce their risk for genital HPV?

A vaccine called Gardasil has recently become available. It is protective against four of the HPV strains most commonly linked to genital warts and cervical cancer in women. The vaccine is administered as a series of three shots and is given to women only.

The surest way to eliminate risk for genital HPV infection is to refrain from any genital contact with another individual.

HPV infection can occur in both male and female genital areas that are covered or protected by a latex condom, as well as in areas that are not covered. While the effect of condoms in preventing HPV infection is unknown, condom use has been associated with a lower rate of cervical cancer, an HPV-associated disease.

During a chemical peel, chemical solutions of varying strengths are carefully brushed onto the skin. The chemicals remove skin cells on the surface of the skin and stimulate the production of new skin cells below. This leaves the skin smoother, less wrinkled, and more youthful in appearance.

Chemical peels can be used effectively for a wide range of skin conditions including:

Fine lines and wrinkles
Uneven pigmentation
Shallow acne scars
Sun-damaged skin
Age spots
Freckling

Chemical peeling can be performed on the face, neck, chest, hands, arms, or legs. It is most effective at changing the appearance on the surface of the skin. Effects of aging in deeper tissues, such as drooping eyelids and loose skin below the chin, must be treated with other procedures.

Chemical peels are categorized by how deeply the chemicals penetrate the skin: superficial, medium, or deep. Superficial peels use mild chemicals, such as alpha-hydroxy acids, which penetrate only the uppermost layer of the epidermis. Medium and deep peels use stronger agents.

A superficial chemical peel is usually sufficient to treat fine lines and wrinkles around the eyes and mouth. Patients with minimal skin damage often respond best to a series of superficial peels combined with a skin care regimen, including topical retinoids and daily sunscreen use.

A medium-depth peel is more effective for patients with moderate skin damage, including age spots, freckles, and actinic keratoses. It may be combined with another treatment such as laser resurfacing for maximum effectiveness.

Deeper peels may require bandages to be applied for 1-3 days following treatment. In addition, ointments and creams may be recommended to keep the skin moist. Cosmetics can be applied after about two weeks.

During treatment, most patients experience a warm sensation for about ten minutes, followed by some stinging. Anesthesia may be recommended for deep peels.

In general, the deeper the peel, the longer the recovery. A superficial peel produces redness similar to mild sunburn and lasts 3 to 5 days. Medium or deep peels can make the skin look severely burned, sometimes with blistering and peeling for 7 to 14 days. You may be prescribed pain relievers to reduce the discomfort following a deep peel.

Following a treatment, treat your skin gently. Avoid sun exposure because the renewed skin will be tender and susceptible to injury.

Following a chemical peel, your new skin will be tighter, smoother, and perhaps slightly lighter in color than before surgery.

Facial lines and wrinkles

Fine lines and wrinkles are signs of intrinsic or chronological aging; there's no way to entirely avoid them. As we grow older, our skin becomes thinner and drier. Weakened collagen makes the skin less elastic. Static wrinkles begin to appear. The rate of intrinsic aging occurs at a variable, genetically determined rate; the process is often first noticeable between the ages of 30 and 35.

We do have control over extrinsic aging, a result of exposure to the environment. Extrinsic aging is the critical element in determining who looks older or younger than their biological age. Over-exposure to sunlight is responsible for the majority of problems associated with extrinsic aging. Sun-damaged skin is photoaged from a decrease in collagen and other dermal proteins. This gradual process results in the appearance of fine lines and wrinkles. Over time, these can progress into deep wrinkles and furrows. Photoaging also causes pigmentary changes; "age spots" (sun-induced freckles), uneven skin tone, spider veins and dilated capillaries.

The simplest way to see the contrast between intrinsic and extrinsic aging is to compare skin commonly exposed to the sun (face, hands, neck) and skin not exposed to the sun (usually on the breasts, inner arms or buttocks).

There are a number of options for the treatment of fine lines and wrinkles, each with varied expense, recovery time and results. The following are the most common conventional treatments used today:

Nonprescription and Prescription Topicals: The simplest method of skin treatment is the topical application of wrinkle creams. Widely available everywhere from pharmacies to health food stores, these creams are beneficial for the skin, helping to moisturize and nourish it. However, their ability to reduce wrinkles below the skin's surface is, unfortunately, very limited. In addition, any visible improvement may disappear when use of the topical agent is discontinued. Topical application of prescription drugs, most notably synthetic derivatives of Vitamin A (retinoids), can provide better results in improving sun-damaged and aging skin. Clinical studies of topical retinoid formulations find them to be safe and effective in diminishing fine wrinkles. However, most retinoids are skin irritants and often produce skin reactions.

Dermabrasion and Microdermabrasion: Dermabrasion is a traditional skin-clearing procedure that has been widely used in practice since the early 1980s. Sometimes called surgical skin planing, it commonly involves removal or sanding of the skin with a rotary abrasive instrument. The abrasive or planing action evens out the skin, and a new layer of fresh skin replaces the abraded skin. This procedure is usually reserved for extensive acne scarring and deep wrinkles. Due to its mechanical nature, healing and recovery are lengthy, with the treated area remaining red and flushed for an extended period of time.

Microdermabrasion, in contrast, is a more recently introduced treatment. It is particularly popular among individuals with mild skin damage seeking to preserve and maintain a youthful complexion. Essentially, microdermabrasion involves brushing the face with sterile micro-particles to abrade or rub off the very top skin layer, then vacuuming the particles and dermabraded skin away. Experts believe the procedure works best to polish and soften fine lines and dull skin texture. In most cases, a minimum of four to five initial treatments, seven to ten days apart, are necessary to achieve good results.

Chemical Peels: Another major advance in warding off the ravages of age was the advent of the chemical peel. In this treatment, a chemical solution is applied to the skin, causing it to separate and peel off. The new, regenerated skin is usually smoother, tighter and displays a more even color than the old skin. Peels are beneficial for fine lines and minor skin imperfections, and are performed as a single procedure or in a series.

Laser Resurfacing Produces Long-Term Results

TUESDAY, July 22 (HealthDay News) — Carbon dioxide laser resurfacing still seems the best way to erase telltale signs of aging on your face — and keep them away.

The technique does have a fairly high rate of complications, namely lightening of the skin color, according to authors of a study published in the July/August issue of Archives of Facial Plastic Surgery.

The finding is not entirely new. But, said Dr. Keyvan Nouri, director of dermatologic and laser surgery at the University of Miami Miller School of Medicine, "The point is basically that carbon dioxide is still a very good treatment. It does have a down time and some side effects, but it does cause a dramatic improvement in many patients that we can't achieve with anything else out there now."

Added study co-author Dr. Shan R. Baker, director of the Center for Facial Cosmetic Surgery at the University of Michigan Medical Center: "There's nothing earth-shattering about this paper except the fairly long-term follow-up. It just confirms that carbon dioxide laser resurfacing is very effective for deeper facial wrinkles, and it's long-lasting."

The laser works by absorbing water inside and outside of sells, causing heat damage to nearby tissue. As a result, the skin produces more of the protein collagen, filling in wrinkles.

"It basically causes a controlled burn to the skin," Nouri explained. "Then it removes the epidermis [top layer of the skin] and dermis [middle layer of the skin]. It also causes contraction or tightening of the skin."

The authors of the study described the experiences of 47 patients who underwent full-facial carbon dioxide laser resurfacing at the University of Michigan. Forty-two of the patients were women; the average age was 52.

After more than two years of follow-up, participants' "wrinkle scores" improved 45 percent and were consistent across the face.

The main long-term complication was hypopigmentation (loss of skin color), notably present in 13 percent of patients. One case of hyperpigmentation (patches of skin that become darker) righted itself within two years of treatment.

Some patients also developed milia, small cysts or acne. One participant developed an infection, and one had sagging of the eyelids.

"The biggest problem with CO2 is lightening of the skin. Essentially all of our patients got lightening to some degree, but a certain percentage got marked lightening where you could see quite a bit of difference on photographs," Baker said. "That's the cost of getting rid of wrinkles permanently or near-permanently."

"Part of it has to do with how aggressive you are with peeling," he added. "If there are more wrinkles, you have to do more energy levels, and you often do more than two or three passes, so the patients are going to get more lightening. The deeper you go, the more lightening you are going to get."

According to Nouri, today's treatments tend to be less aggressive than in the past, so healing is faster.

The lightening tended to be consistent across the face, but there was a demarcation between the color of the skin of the face and that of the neck.

"That's why we peel down below the jaw line to try to hide it in the shadow," Baker said.

"How do Chemical Peels work?

There are basically four major types of chemical peel solutions:

Beta Hydroxy (Salicylic Acid)
Cosmelan
TCA (Trichloroacetic Acid)

Chemical peels work by "ungluing" the top layer of damaged and discolored skin so it can peel away and reveal the fresh new skin beneath. By forcing the turnover of the cells, the old dull and damaged cells peel off much more quickly.

Lighter superficial peels using Alpha Hydroxy acids such as Glycolic Acid can be very effective in a series of "no downtime" procedures. Your skin will have a light pink look and flaking or peeling may be unnoticeable. Superficial peels are ideal for improving pigmentation problems, fine wrinkles and sun damage. They can be used anywhere on the face and body.

Special peels containing salicylic acid (beta hydroxy acid) are especially effective for those suffering from acne and oily skin. What will a chemical peel feel like?

Dr. Rothfeld, a board certified dermatologist at NYC Dermatology will first clean your skin of any oils. The chemical solution is either brushed on or applied with a pad. You may feel a slight stinging as the peel solution penetrates your skin.

Hair loss

Loss of hair; Alopecia; Baldness

Partial or complete loss of hair is called alopecia.

According to Dr. Rothfeld at NYC Hair Loss Center, hair loss usually develops gradually and may be patchy or diffuse (all over). Roughly 100 hairs are lost from your head every day. The average scalp contains about 100,000 hairs.

Each individual hair survives for an average of 4-1/2 years, during which time it grows about half an inch a month. Usually in its 5th year, the hair falls out and is replaced within 6 months by a new one. Genetic baldness is caused by the body's failure to produce new hairs and not by excessive hair loss.

Both men and women tend to lose hair thickness and amount as they age. Inherited or "pattern baldness" affects many more men than women. About 25% of men begin to bald by the time they are 30 years old, and about two-thirds are either bald or have a balding pattern by age 60.

Typical male pattern baldness involves a receding hairline and thinning around the crown with eventual bald spots. Ultimately, you may have only a horseshoe ring of hair around the sides. In addition to genes, male-pattern baldness seems to require the presence of the male hormone testosterone. Men who do not produce testosterone (because of genetic abnormalities or castration) do not develop this pattern of baldness.

HAIR LOSS

ABOUT HAIR LOSS IN MEN

According to Dr. Gary Rothfeld, a board certified dermatologsit at the NYC Hair Loss Center male hair loss is the most common type of hair loss. It is caused by increased sensitivity to male sex hormones (androgens) in certain parts of the scalp, and is passed on from generation to generation.

Some men have areas on the scalp that are very sensitive to the male sex hormones that circulate in men's blood. The hormones make the hair follicles - from which hair grows - shrink. Eventually, they become so small that they cannot replace lost hairs. The follicles are still alive, but are no longer able to perform their task.

The condition usually starts in men aged 20 to 30 and follows a typical pattern. First, a receding hairline develops, and gradually the hair on top of the head also begins to thin. Eventually, the two balding areas meet to form a typical U-shape around the back and sides of the head. The hair that remains is often finer, and does not grow as quickly as it used to.

Some women also develop a particular pattern of hair loss due to genetics, age, and male hormones that tend to increase in women after menopause. The pattern is different from that of men. Female pattern baldness involves a thinning throughout the scalp while the frontal hairline generally remains intact.

ABOUT HAIR LOSS IN WOMEN

Unlike hair loss in men according to Dr. Gary Rothfeld at NYC Cosmetic Dermatology and Hair Loss Center female scalp hair loss may commonly begin at any age through 50 or later, may not have any obvious hereditary association, and may not occur in a recognizable "female-pattern alopecia" of diffuse thinning over the top of the scalp. A woman who notices the beginning of hair loss may not be sure if the loss is going to be temporary or permanent—for example, if there has been a recent event such as pregnancy or illness that may be associated with temporary hair thinning.

In women as in men, the most likely cause of scalp hair loss is androgenetic alopecia—an inherited sensitivity to the effects of androgens (male hormones) on scalp hair follicles. However, women with hair loss due to this cause usually do not develop true baldness in the patterns that occur in men—for example, women rarely develop the "cue-ball" appearance often seen in male-pattern androgenetic alopecia.

Patterns of female androgenetic alopecia can vary considerably in appearance. Patterns that may occur include:

-Diffuse thinning of hair over the entire scalp, often with more noticeable thinning toward the back of the scalp.

-Diffuse thinning over the entire scalp, with more noticeable thinning toward the front of the scalp but not involving the frontal hairline.

-Diffuse thinning over the entire scalp, with more noticeable thinning toward the front of the scalp, involving and sometimes breaching the frontal hairline.

Unlike the case for men, thinning scalp hair in women due to androgenetic alopecia does not uniformly grow smaller in diameter (miniaturize). Women with hair loss due to androgenetic alopecia tend to have miniaturizing hairs of variable diameter over all affected areas of the scalp. While miniaturizing hairs are a feature of androgenetic alopecia, miniaturization may also be associated with other causes and is not in itself a diagnostic feature of androgenetic alopecia. In post-menopausal women, for example, hair may begin to miniaturize and become difficult to style. The precise diagnosis should be made by a physician hair restoration specialist.

It is important to note that female pattern hair loss can begin as early as the late teens to early 20s in women who have experienced early puberty. If left untreated, this hair loss associated with early puberty can progress to more advanced hair loss if it is left untreated

Common Causes

Baldness is not usually caused by a disease, but is related to aging, heredity, and testosterone. In addition to the common male and female patterns from a combination of these factors, other possible causes of hair loss, especially if in an unusual pattern, include:

Hormonal changes (for example, thyroid disease, childbirth, or use of the birth control pill)
A serious illness (like a tumor of the ovary or adrenal glands) or fever
Medication such as cancer chemotherapy
Excessive shampooing and blow-drying
Emotional or physical stress
Nervous habits such as continual hair pulling or scalp rubbing
Burns or radiation therapy
Alopecia areata -- bald patches that develop on the scalp, beard, and, possibly, eyebrows. Eyelashes may fall out as well. This is thought to be an immune disorder.
Tinea capitis (ringworm of the scalp)

Home Care

Hair loss from menopause or childbirth often returns to normal 6 months to 2 years later.

For hair loss caused by illness (such as fever), radiation therapy, or medication use, no treatment is necessary. The hair will usually grow back when the illness has ended or the therapy is finished. A wig, hat, or other covering may be desired until the hair grows back.

For hair loss due to heredity, age, and hormones, the topical medication Rogaine (minoxidil) can be helpful for both male and female pattern baldness. Expect to wait 6 months before you see results. The oral medication Propecia (finasteride) is effective in some men. This medicine can decrease sex drive. When either medication is stopped, the former baldness pattern returns.

Hair transplants performed by a physician is a surgical approach to transferring growing hair from one part of the head to another. It is somewhat painful and expensive, but usually permanent.

Hair weaves, hair pieces, or changes of hair style may disguise hair loss. This is generally the least expensive and safest approach to hair loss. Hair pieces should not be sutured to the scalp because of the risk of scars and infection.

You are losing hair rapidly or at an early age (for example, teens or twenties).
You have any pain or itching associated with the hair loss.
The skin on your scalp under the involved area is red, scaly, or otherwise abnormal.
You have acne, facial hair, or menstrual irregularities.
You are a woman and have male pattern baldness.
You have bald spots on your beard or eyebrows.
You have been gaining weight or have muscle weakness, intolerance to cold temperatures, or fatigue.

Are you losing hair only from your scalp or from other parts of your body as well?
Is there a pattern to the hair loss like a receding hair line, thinning or bald areas on the crown, or is the hair loss throughout your head?
Have you had a recent illness or high fever?
Do you dye your hair?
Do you blow dry your hair? How often?
How often do you shampoo your hair?
What kind of shampoo, hair spray, gel, or other product do you put on your hair?
Have you been under unusual stress lately?
Do you have nervous habits that include hair pulling or scalp rubbing?
Do you have any other symptoms like itching, flaking, or redness of your scalp?
What medications do you take, including over the counter drugs?

Diagnostic tests that may be performed (but are rarely needed) include:

Microscopic examination of a plucked hair
Skin biopsy (if skin changes are present)

Ringworm on the scalp may require the use of an oral drug, such as griseofulvin, because creams and lotions applied to the affected area may not get into the hair follicles to kill the fungus.

Treatment of alopecia areata may require topical or injectable steroids or ultraviolet light.

Acne is one of the most common of all skin problems. It affects most teenagers to some degree and even more adults. Acne shows up as whiteheads, blackheads, pimples and deep painful bumps that look like boils. Acne most commonly occurs on the face but can also appear on the neck, back, chest and shoulders. It is estimated that as many as 80% of people between 12 and 25 years of age suffer from acne. While in most people, acne clears up after few years, many suffer from permanent scarring of the skin. Even when there are few physical marks left, the emotional ones can be devastating.

Acne problems are commonly faced by many young adults and teenagers today. Some people develop acne even at forties and fifties. According to Dr. Rothfeld at NYC Dermatology acne is believed to develop due to various factors like hormonal imbalance, sebum secretion, hereditary factors, and food habits.

There are many treatment options for adult acne problems, but it is not mandatory that everyone gets cured. Dermatologists prescribe medicines taking skin care into concern. In many cases, blood and urine tests are taken, to test for any underlying causes. Severe acne patients are given antibiotics and very severe adult acne patients are treated with laser surgery, considering the seriousness of the problem. Types of Acne Scars

It is common to have stains and mild scars from acne. The FDA recently approved the Fraxel laser for the treatment of acne scarring. First introduced in 2004 for the treatment of age-related wrinkles and sun-damage, this laser works by gently “resurfacing” the skin and replacing the damaged goods with new, healthy tissue.

Most acne scar patients need only three to four treatments. There’s minimal downtime, if any at all, according to researchers at Stanford University who conducted studies on the Fraxel.

FDA Proposes New Sunscreen Rules

Proposed Sunscreen Regulation Includes a Rating System, New Warning

Aug. 23, 2007 -- Sunscreens may get a new rating system and a warning, thanks to a new FDA proposal.

The FDA today proposed a new regulation that sets standards for formulating, testing, and labeling over-the-counter sunscreens with ultraviolet A (UVA) and ultraviolet B (UVB) protection.

The proposal includes a four-star rating system for sunscreen protection against UVA light, along with a warning that stresses the importance of not relying solely on sunscreens to prevent skin cancer and skin damage.

Sunlight contains the visible light that we can see and ultraviolet (UV) light that we cannot. There are two types of UV light, UVA and UVB. UVA light is responsible for tanning and UVB for sunburn. Both can damage the skin and increase the risk of skin cancer.

SPF ratings on sunscreens refer only to UVB protection, not UVA protection. For years, the FDA has been studying UVA's effects on skin, but until now hasn't required sunscreens to carry specific information on UVA protection.

"Only a few countries have addressed UVA protection in their product labeling," says Douglas Throckmorton, MD, deputy director of the FDA's Center for Drug Evaluation and Research.

The proposed new regulation would include cosmetics that tout sun protection but not insect repellents that include SPF, according to Matthew Holman, PhD, interdisciplinary scientist team leader at the FDA's Office of Nonprescription Products.

New UVA Sunscreen Ratings

The FDA's proposed sunscreen regulation is designed to be a consumer-friendly rating system for identifying the level of UVA protection offered by a product. It rates UVA sunscreen products on a scale of one to four stars.

One star would represent low UVA protection, two stars would represent medium protection, three stars would represent high protection, and four stars would represent the highest UVA protection available in an over-the-counter sunscreen product.

If a sunscreen product does not provide at least a low level (one star) of protection, the FDA is proposing to require that the product to bear a “no UVA protection” marking on the front label near the SPF value.

The UVA ratings would be based on two tests the FDA proposes to assess the effectiveness of sunscreens in providing protection against UVA light. One would be a lab test that measures a product's ability to reduce the amount of UVA radiation that passes through it. The other test would be conducted on people, checking the product's ability to prevent tanning. This test is nearly identical to the SPF test used to determine the effectiveness of UVB sunscreen products.

If a sunscreen scores higher on one test than the other, it would bear the lower rating, says Holman.

The proposal also includes tweaking the SPF information on sunscreens. For instance, a sunscreen that currently says "SPF 45" would say "UVB SPF 45" with the word "high" to indicate a high level of UVB protection.

New Sunscreen Warning

The FDA also proposes requiring all sunscreens to include a "Warnings" statement in the "Drug Facts" box.

The warning would say: "UV exposure from the sun increases the risk of skin cancer, premature skin aging, and other skin damage. It is important to decrease UV exposure by limiting time in the sun, wearing protective clothing, and using a sunscreen." The warning is intended to increase awareness that sunscreens are only one part of a sun protection program.

The proposed new sunscreen regulation will also stress instructions to reapply sunscreen at least every two hours and to use sunscreen as directed, says Holman.

The FDA will accept comments on the proposed new rule for 90 days, ending on Nov. 26, 2007.

Trade Group Responds

The Cosmetic, Toiletry, and Fragrance Association (CTFA) states that it is "pleased that the FDA has proposed these new sunscreen regulations" and that it looks forward to working with the FDA as it finalizes the sunscreen rules.

The CTFA points out that "while the final rulemaking is ongoing, there are currently strong FDA safety and effectiveness regulations that govern the manufacture and marketing of all sunscreen products."

"Consumers should have a high level of confidence that sunscreen products are safe and effective when used as directed," states the CTFA.

Like the FDA, AAD, ACS, and other health experts, the CTFA stresses that besides wearing sunscreen, people should minimize their sun exposure -- especially from 10 a.m. to 4 p.m., when UVA/UVB exposure is highest -- and to wear protective clothing to safeguard skin.

Stretch Marks
Between 75 and 90% of women develop stretch marks to some degree during pregnancy. The sustained hormonal levels as a result of pregnancy usually means stretch marks may appear during the sixth or seventh month.

Many people believe that using oils or creams in high-risk areas (e.g. breasts, hips, abdomen, thighs) may help prepare the skin for stretching by giving the skin more flexibility.

Though cocoa butter is an effective moisturizer, no research studies have shown its ability to either prevent stretchmarks, or improve their appearance once a stretch mark has already formed.

Various treatments are available for the purpose of improving the appearance of existing stretch marks, including laser treatments, chemical peels, dermabrasion, and prescription retinoids. Few published studies exist for over-the-counter remedies, but two ingredients have shown early promise in clinical research-- darutoside and registril [Sederma Labs. Used daily for one month, they resulted in significant improvement in the appearance of a stretch mark's length, depth, and irregular surface area. One available product that contains both darutoside and registril is Stretchmark Minimizing Cream, from Belli Cosmetics.Some cream manufacturers claim the best results are achieved on recent stretch marks; however, few studies exist to support these claims.

A recent study in the journal "Dermatologic Surgery" has shown that radiofrequency combined with 585-nm pulsed dye laser treatment gave "good and very good" subjective improvement in stretch marks in 89.2% of 37 patients, although further studies will be required to follow up on these results. In addition, the use of a pulsed dye laser has shown to increase pigmentation in darker skinned individuals with repeated treatments.

A surgical procedure for removing lower abdominal stretch marks is the tummy tuck, which removes the skin below the navel where stretch marks frequently occur .But the tummy tuck can leave you with hypertrophic scars which could be flattened with intralesional steroid injections.

A new modality, fractional laser resurfacing, offers a novel approach to treating striae. Using scattered pulses of light only a fraction of the scar is zapped by the laser over the course of several treatments. This creates microscopic wounds and as such is a "no downtime" procedure. The body responds to each treatment by producing new collagen and epithelium. Intercytex licenses new cell storage technology

InterCytex has signed a licensing agreement with BioLife Solutions Inc., a leading developer and marketer of proprietary hypothermic storage and cryopreservation media products for cells and tissues.

Terms of the 10-year agreement include an intellectual property escrow provision which guarantees Intercytex access under certain conditions to BioLife’s HypoThermosol storage and preservation media when used in the production of Intercytex’ VAVELTA (ICX-RHY), a facial rejuvenation product and ICX-TRC, their hair regeneration product, as well as annual license fees payable to BioLife.

New patent issued to Aderans for hair cloning technology

Aderans Research Institute, Inc., has just been issued a patent for their bioabsorbable scaffolds. These scaffolds are used in hair cloning procedures to ensure that the cultured cells are kept in place in order for a new hair follicle to be formed.

The patent, number 7,198,641, which was initially filed on August 7, 2001 and has subsequently been revised, was finally issued on April 3, 2007.

According to the patient, the “bioabsorbable scaffolds are useful for the tissue engineering of new hair follicles and to methods for their manufacture and to methods of their use in creating new hair. More specifically it relates to new and useful bioabsorbable porous structures that have the correct architecture to facilitate culturing of the appropriate follicle progenitor cells and their development into normal, functional, hair-producing follicles. The invention also relates to methods of making and using bioabsorbable scaffolds to implant and grow new hair follicles in vitro and in vivo.”

These tiny scaffolds are essentially the key to being able to grow new hair follicles because without them the cells cannot be held together after being injected into the scalp. New hair follicles get created in a specific process that results from the interaction between different cells. If these cells cannot be held together in the same place by some mechanism, then they will simply disperse from the point of injection and no new hair follicle will be formed.

There’s nothing worse than looking older than your age but you might be surprised to learn there are some free, easy ways to prevent you looking your age…

A curious fact about ageing is that many of the signs of ageing we later learn to dread like fine lines, pigmented skin, loss of elasticity, hair greying and thinning don’t rear their ugly faces until MANY years after the behaviours that gave rise to them. Take smoking for instance. We can smoke and smoke and smoke in our teens and still look hot and feel we are getting away with it only to discover in our mid twenties that we look 5 -10 years older than our friends. What you do now will show up on your face many years later…

So what can we do now to keep looking younger later?

The first thing we can do is recognize our enemies… sunlight, alcohol, fatty foods, smoking and last but not least caffeine. As we shall see, each damages us in different and not always predictable ways.

Sunlight

Ahhh summer. Nice isn’t it? Not if you want to avoid premature ageing! We all know UV damages our skin and gives us cancer blah, blah, blah but what we might not know is why UV exposure is so damaging. Basically your face looks as it good as it does when you’re young because it contains lots of collagen which keeps your skin nice and elastic. When your skin loses that elasticity lines begin to develop and we start to look old.

Probably a major reason we don’t take UV seriously is that we can’t see it; UV has a wavelength shorter than that of visible light. Worse UV comes in several forms. UVA for long wavelength, UVB or medium wavelength and UVC or short wavelength. When we talk about UV we usually are talking about UVB, it’s what the SPF factor on your sunscreen protects you against. So what you ask? Well sunscreen only protects you against UVB and only moderately against UVA. This is because UVB can be easily measured but UVA can’t. This matters because UVA penetrates deeper into your skin than UVB and is believed to be a major contributor to the emergence of lines and wrinkles.

So what can we do? Well the best thing to do is STAY out of the sun completely but then you might suffer from vitamin D deficiency which isn’t good either. Realistically avoid the sun between 10am-4pm if you can. Big hats are a good idea but are hard to pull off without looking sad. Wear sunscreen everyday and reapply every 2 hours Why? Your sunscreen dissipates with sweat and can vary in its coverage after that dip in the sea. Wear sunscreen daily even to work since UV penetrates the glass in your car whilst you’re driving to work. UV laughs at cloudy days since clouds DO NOT reduce UV exposure so not wearing sunscreen on that overcast day isn’t doing you any good.

Avoid ski slopes. Just joking but reflections from surfaces still will burn and age your skin so sailors for instance should always wear sunscreen to reduce reflected UV light off the water. Slopping on that t shirt does very little to UVB and UVA light. Better to buy a sunscreen with both UVB and UVA protection. Such sunscreens will have compounds like titanium oxide (sounds cool doesn’t it?) that prevents UVA light penetrating your skin.

Avoid tanning salons. Really! Tanning won’t make you any prettier just a whole lot older later in life when you really want to hide your age.

Skin colour plays a role too. If you’re dark or olive skinned take a bow… the sun will do much less damage to your skin but if your blond, blue eyed or red haired, watch out, you’re fair skin makes you extra vulnerable to damage from UV. That tan you think makes you look sexy is your body releasing melanin to darken your skin and protect it from UV damage. Problem is if you tan too much and you’re fair skinned, you won’t just burn but will be creating little spots (excess melanin) that darken with time and emerge more and more as we age. Worse people call them “age” or “liver” spots. Effectively you are tanning yourself towards freckles. Sounds hot doesn’t it?

Alcohol

The reason many of us went out with… well lets not go there… Fun as drinking is, too much of it can also make you look old and fast! Like it or not there is a clear and proven link between alcohol and premature ageing. Drink enough and you will damage blood vessels in the skin giving you that much desired red flushed look. Even worse keep drinking excessively and broken blood vessels will start appearing on your skin giving you a red mottled complexion. Hot! So how much is too much? I hope your seated…take a deep breath, your not going to like this – just one drink a day for the ladies and up to two drinks a day for guys. No one said it would be easy but think how much money you’ll save!

Smoking

It’s funny how a lot of us started smoking to look older and that exactly what smoking will do to your appearance. Basically smoking puts a fine layer of filth on your skin. Without this being removed it contributes to premature ageing to say nothing of smokers’ lines around your mouth, a sickly yellow complexion, increased wrinkling, risk of cancer and the fact that if your about 16 you just look like a try hard. As they say be careful what you wish for, you might just get it!

Fatty foods

These basically make you fat and whilst being overweight won’t necessarily make you look older often it does. Why? Well when you’re young ever notice that you don’t have big cheeks or a line or crease running from your nose to each corner of your mouth in a broadly triangular shape? This is called the nasiolabial fold and most us get them as we get older. In fact it’s a major difference between looking under 25 or over 25. Unless you’re overweight as a child you won’t have then till you’re in your mid twenties. Some people actually look better with this line but most don’t
which might explains why that spunk you met all those years ago doesn’t look very nice now. Nasiolabial folds really do dramatically change the way you look. They can be avoided. Keeping the weight off helps eliminates this line and eliminating these lines makes us look young. Don’t diet too much though or the line will appear as well, just don’t be overweight or too thin. Avoiding sunlight helps too since the loss of collagen from UV exposure is a major factor in the emergence of this line.

Still don’t believe me? Well a recent 2005 study showed wait for it, that someone who smokes and is overweight at 30 can feel and LOOK like a 40 year old. So avoid those fatty foods.

Caffeine
There’s little hard evidence that caffeine does you any harm but look at it a different way. No one denies lack of sleep ages you, hence the phrase “I’m getting in by beauty sleep”. Caffeine hurts you by letting you cut corners. Where you might just go to sleep after a busy week, caffeine lets you hit the town and go to work 4 hours later. Sounds good doesn’t it? Well its not. Your skin needs time to rejuvenate, shed old skin cells and replace them; that’s what sleep is for, to repair your body. Don’t sleep. Don’t repair. That’s why models sleep as much as possible; keeps them looking young, avoids panda eyes- you get the idea…

Caffeine can dehydrate us and this is very bad for our skin. Again that’s why models drink lots of water. It cleanses your body, makes your skin glow; caffeine does the opposite. Finally no one denies stress visibly ages you. Well, caffeine elevates cortisol so avoiding caffeine can help you reduce your stress and this reduces ageing. Yoga can help too. Clears the mind you see, lets you relax…

So if we are serious about looking young, cut out the caffeine and sleep when you’re tired. Still want to party? Go ahead but take a nap like your grandpa first. Don’t tell anyone; they’ll laugh but if you sleep before you party you will look better than your friends as it gets later in the night and you will look better in the morning too. You will be amazed how refreshed you can feel at say 4 in the morning; try it you will be surprised.

So there you have it… Don’t drink. Avoid the sun, don’t eat any fats, no caffeine and don’t smoke. Doesn’t sound like much fun does it? But I bet partying into your mid twenties when all your mates look thirty doesn’t sound too bad either. It won’t be easy but think of how much money you will save (no drinking, smoking, coffee) and how nice it would be to still have “options” rather than settling with what you have because you look too old to get better…

True or False? Melanomas cannot occur in areas of skin not exposed to the sun.

It is true that excessive sun exposure, especially multiple sunburns, can cause melanoma. In addition, there is a genetic component to melanoma development, and melanomas can occur on ANY area of the skin. It is recommended that you do regular self-skin checks to look for any changes in your moles. A yearly skin check with your dermatologist is also advised.

NON- SURGICAL NOSE JOB

People who are thinking about getting their noses done but are afraid of surgery need not worry. There is now a non-surgical nose job. In fact, DR. The procedure involves the use of a needle rather than a scalpel. Filler such as Radiesse or Restylane are used along with the needle to shape and contour the nose. This includes small bumps and little bulges. If the defect is a major nasal asymmetry and irregularities, then it is better for the patient to go through a rhinoplasty procedure. Furthermore, the non-surgical nose job cannot change the shape of the nostrils.

Dry skin and Itching

Many older people suffer from dry skin, particularly on their lower legs, elbows, and forearms. The skin feels rough and scaly and often is accompanied by a distressing, intense itchiness. Low humidity — caused by overheating during the winter and air conditioning during the summer — contributes to dryness and itching. The loss of sweat and oil glands as you age also may worsen dry skin. Anything that further dries your skin — such as overuse of soaps, antiperspirants, perfumes, or hot baths — will make the problem worse. Dehydration, sun exposure, smoking, and stress also may cause dry skin.

Dry skin itches because it is irritated easily. If your skin is very dry and itchy, see a doctor. Dry skin and itching can affect your sleep, cause irritability, or be a symptom of a disease. For example, diabetes and kidney disease can cause itching. Some medicines make the itchiness worse

The most common treatment for dry skin is the use of moisturizers to reduce water loss and soothe the skin. Moisturizers come in several forms — ointments, creams, and lotions. Ointments are mixtures of water in oil, usually either lanolin or petrolatum. Creams are preparations of oil in water, which is the main ingredient. Creams must be applied more often than ointments to be most effective. Lotions contain powder crystals dissolved in water, again the main ingredient. Because of their high water content, they feel cool on the skin and don’t leave the skin feeling greasy. Although they are easy to apply and may be more pleasing than ointments and creams, lotions don’t have the same protective qualities. You may need to apply them frequently to relieve the signs and symptoms of dryness. Moisturizers should be used indefinitely to prevent recurrence of dry skin.A humidifier can add moisture to the air. Bathing less often and using milder soaps also can help relieve dry skin. Warm water is less irritating to dry skin than hot water. If a patient has this type of problem they should be evaluated by our Board Certfied Dermatologist at NYC Dermatology to make sure they are getting the proper therapy and also if it is not associated with something more serious.

Sun Protection

NYC attracts many people because there many days of each year of blue skies and sunshine. While it is wonderful that we can enjoy the outdoors and get some exercise (hopefully!) in the process, we also need to be aware of the long term affects of the sun. Here is what you need to know about sun protection to avoid being one of the 500,000 people in this country each year who are diagnosed with skin cancer.

Enjoy the Sun
When going outside always use a sunscreen. The higher the SPF rating of the sunscreen, the longer you can stay out before re-applying the sunscreen.

What is SPF?
SPF is an acronym for Sun Protection Factor. Take the amount of time it would take to burn without sunscreen (UV Index) and multiply it by the sunscreen's Sun Protection Factor to figure out how long you can be outside with sunscreen. For example, if it will take 15 minutes to burn today without sunscreen, and you use an SPF 8 product, you can say outside 2 hours without burning (8 x 15= 120 minutes or 2 hours).

Is It That Simple?
No, of course it isn't! Sun Protection Factor numbers serve as a guideline. How sunscreen affects and protects you that depends on your skin type, the strength of the sunlight, the type of sunscreen you use (gel, cream, lotion, or oil), and the amount you apply. In general, don't be skimpy when applying your sunscreen, and reapply it after you have been sweating or swimming.

What If I Have Blue Eyes?
People who sunburn easily are more likely to develop skin cancer. If you have blue eyes, blonde hair, red hair or get freckles in the sun, you are at greater risk and should take even more care to protect your skin from the sun. And remember--90% of all skin cancers occur on parts of the body that are not protected by clothing like your face, ears and hands.

When Is the Sun Most Dangerous?
In sunny climates you are at the greatest risk for sunburn and need the most sun protection between 10 a.m. and 3 p.m. If you happen to be outside on one of NYCs' cloudy days, don't think you are safe from the sun! Will I Die If I Get Skin Cancer?
There are 3 types of skin cancer:

Basal cell carcinoma is the most common and it is likely that your doctor can cure it.
Squamous cell carcinoma, if detected early, can be cured.
Melanoma is the worst and could result in death. With a family history of melanoma, you could get it even if you've never been out in the sun.

It may look good now, but spending too much time in the sun without sun protection and burning your skin will, at best, age your skin prematurely, and at worst, lead you down the path to skin cancer. Description

A malignant growth or tumor on the skin that tends to grow and spread. There are three basic types of skin cancer, described as follows:

Nodular Basal Cell Carcinoma

Basal cell carcinoma, occurring mostly on the face, neck and hands, is usually a slow-grower and may be scaly, flat or raised. It often crusts and bleeds, but does not heal or go away completely.

Squamous cell carcinoma, typically appearing on the face, hands and ears, appears as a reddish/pink scaly bump. It may begin as a pink or red crusted area.

Malignant melanoma is the most serious of skin cancers. It usually begins as a light brown or flat, black spot with irregular borders that later can become red, blue or white. It may start in an existing mole or as a new flat freckle with irregular borders.

According to the AAD, over exposure to ultraviolet light (from the sun and indoor tanning lights) is the greatest risk factor for skin cancer. They state that ultraviolet radiation causes almost all cases of basal and squamous cell skin cancer, and sun exposure is a major cause of skin cancer.

Excessive sun exposure during the early years (birth through age 18) increases the risk of melanoma by threefold. Caucasians are 20 times more likely than African-Americans to develop malignant melanoma. Fair-skinned people have twice the risk of developing melanoma as olive-skinned people. Redheads and blondes have a two to four time increased risk of developing melanoma.

There is an increased risk of nine times for individuals that have already had one melanoma. People with many or large moles on their bodies have a substantially higher risk of melanoma. Those with a history of melanoma in their family have a higher risk of developing melanoma themselves.

One out of every five Americans will develop some form of skin cancer in their lifetime. Skin cancer is the most common type of cancer. Half of all cancers diagnosed is skin cancer. It is estimated that 1 million new cases of skin cancer will be diagnosed this year in the United States. Of these, 80% will be basal cell carcinoma, 16% will be squamous cell carcinoma and 4% will be melanoma.

In 1998 it is estimated, 1 in 82 Americans have a lifetime risk of developing melanoma. This increases to 1 in 75 by the year 2000. In 1998 new cases of melanoma increased by 3% over 1997.

Basal cell and squamous cell carcinoma have a better than 95% cure rate if detected and treated early.

What Can Be Done?

Nodular Melanoma

The most important way to reduce the risk of some types of skin cancer is to reduce the exposure to ultraviolet light and radiation.

This can be done by avoiding the sun between the hours of 10 a.m. and 4 p.m. (This is not always a practical or realistic solution).

Wear protective clothing - long-sleeves, hats, sunglasses. (This is not always a practical or realistic solution).

Many different types of surgical procedures are used to remove melanomas and most basal and squamous cell carcinomas. The treatment chosen depends upon the type of skin cancer, the area of the body affected, the depth of the lesion, and the experience and training of the skin cancer doctor involved.
